Please run "smartctl -a" on each boot. Doing it from /etc/init.d/boot.local should do the trick. After the problem occurs, run "smartctl -a" again and attach (please don't post inline) the two outputs. Also, can you please post full boot log? And SL111 in some cases do require more power if you're on raid. SL111 raid code handles barriers correctly and will issue FLUSH_CACHE to all disks in an array simultaneously, which caused brief disk power loss and data loss on certain configurations.
