There was a post in suse-kde about this same problem, but it related to errors in /var/log/messages with problems on that system writing to /var/run/xdmctl/xdmctl. I don't have that error message.

Make sure the file $KDE_DIR/share/config/kdm/Xservers contains entries to configure additional X sessions; the lines may be there but commented out.
Mine looks like that: :0 local@tty1 /usr/X11R6/bin/X -nolisten tcp :0 :1 local@tty2 reserve /usr/X11R6/bin/X -nolisten tcp :1 :2 local@tty3 reserve /usr/X11R6/bin/X -nolisten tcp :2 :3 local@tty4 reserve /usr/X11R6/bin/X -nolisten tcp :3
Also have a look at /etc/X11/xdm/Xservers, where there should be similar entries.

I do have a similar entry in /etc/X11/xdm/Xservers, but only one, an "0" entry.
I'm not sure where to find $kde_dir/share/config/kdm/Xservers. Is there a file in there that I can search for?

If you are running suse 9.0 or 9.1 then $kde_dir/share/config/kdm/Xservers is actually /opt/kde3/share/config/kdm/Xservers... i must admit i do not know why people cant make it plain for the unshure but still thats there perogative .
