I should have mentioned that KDE was never told nor set up to automatically login on start up.

I suppose you mean "the screen is not locked when coming back from suspend"? That's different from "automatically logs in"...
For me, that would be accurate. The screen does not lock when the computer comes back from suspend (laptop lid closing). I also did not have the "Automatic Login" box checked. What other people? If somebody has a different problem, they should report a separate bug anyway.
Is there an easy way to check if Auto-Login is enabled?
grep AUTOLOGIN /etc/sysconfig/displaymanager Or YaST->Securtiy and Users->User and Group Management->Expert Options->Login Settings. But again, Autologin is totally unrelated to suspend/resume and only affects a fresh boot. I'm going to assume the other people just did not turn off the Automatic Login option during the install process as I do not have the exact same issue. When I turn my computer on, I get the KDM screen. So, let's put that aside and focus on my specific issue, which is that KDM does not always lock when coming back from suspend. I just tested closing my laptop lid for 5 seconds, both while plugged in and while on battery power. Both times, the KDM login screen popped up and requested my password. There have been times where I close the lid for 5-10 minutes and when I open the lid it does not prompt me for the password. When I turn my computer on, I get the KDM screen. A recommended display manager for Plasma is SDDM now. And to be precise, a lock screen is handled by Plasma. These two things are usually themed in a way that

I had something similar on Debian 9 Stable, with Plasma and SDDM. Not sure if it was suspend to RAM or hibernation (by the way, the bug report also has no information on this, and settings for closing lid can be different). It happened from time to time. Now I have Leap 15.1 on the same laptop with only suspend to RAM configured. Except one thing: The screen is not locked if a right-click context menu is open while you suspend/hibernate. See https://bugs.kde.org/show_bug.cgi?id=383327.
