FWIW, I remember that at some point with some terminal or other I had a problem with the title not changing and then discovered that it would change if I opened another tab in the terminal window. I just gave up and switched to LXterminal, so maybe not much help, but it might be worth experimenting.
I just installed it, and I have replaced two open xfce-terminal with lxterminals. One difference is how each one renders the tabs list.
