First: man init.d
Second: corresponding chapter in the suse admin book (pdf, you have to install it first, very good reading).
Third: sample file "/etc/init.d/skeleton" or skeleton.compat
Warning: don't create the symlinks yourself. Use the appropiate suse way of things.
