The setting (in smplayer) is at Options/Preferences/General/Output drivers.
Well, I just switched back to kde4 and tried the video, and before I made any changes, I tried smplayer and kaffeine again. Both are now fine. --Well, kaffeine's audio is still very noisy. There were two updates involving a dozen or so file between my tries on kde4; I wonder if that had something to do with it. I don't remember any of them having to do with kde, though. In sum, things look good now.
