I'll roll some kde packages with debugging enabled in my buildservice home repo so that we can get some console output from Kopete when it tries to open the webcam device. The other kopete+webcam query from 'J Beris' can also use

1) Can you tell us the make/model of webcam?
Logitech QuickCam Communicate STX. It is on the list http://mxhaard.free.fr/spca5xx.html
Logitech 213 0x046d 0x08d7 Communicate STX
Argh. Taupter (upstream video device support developer) had a run-in with mxhaard (spca5xx author) about several stupid things in his drivers and basically refuses to work around its braindeadedness anymore.
2) Have you tried Kopete from KDE 3?
I just did and it partially works. If I go to the settings in the KDE3 version the camera is listed and I see the picture. There are two things that seem a little strange
1. in the KDE4 version that doesn't work, the Video Device Configuration selection "standard" is blank while in the KDE3 version it is PAL-M
I don't think that field is actually used.
2. in both versions the devices page has only a video tab at the top. Should there be an audio tab too?
No, space is left for an audio tab but since audio support in Kopete is still not mature it's not enabled.
With the KDE3 version I was able to establish a connection with my dad who is using Live Messenger on Windows XP. We could see each other but there was no sound for either of us. While still in KDE3 I tried Ekiga. I ran the setup wizard and all the tests worked. I could see my picture and I could even hear myself in the sound test. This is with the webcam built in mic.
Right, Kopete's MSN plugin doesn't support sound yet. Regarding the 'missing picture in Kopete 4' problem I would just use KDE 3's Kopete for the time being. I'll make those debug packages then we can see what's going wrong. Perhaps Kopete or gideo are crashing the hardware. Perhaps pulseaudio or gstreamer have the video device open so Kopete can't get a picture. Regarding audio: There's not a lot I can do to help - adding audio support to MSN is a big job, so unless a motivated individual picks it up it will continue to be a TODO.
