It seems that openSUSE Leap only has systemd 228. RandomizedDelaySec was added in 229. However, this should just be a warning and should not break execution. Can you check your log for the time the timer ran last? Can you check whether its enabled?
You're right, the timer was not enabled and manually starting it worked. I think my confusion came from the fact that the timer was not enabled but I was using a cron job before and that worked. I can understand the reasons for migrating, but unfortunately it silently broke renewal, which is unexpected for a stable release like Leap.
