I think your guesses are all wrong. Something else seems to be going on. I
may have reached an internal KDE limit.
I can right click on the image (the broken link, actually) and select "copy
image location". That's what I used when opening this bug report.
I can then paste that link into firefox, and it show up the image.
If I paste that link into konqueror, I get:

I've tried this on a different computer, and it is fine.
My best guess -- KDE has an internal limit. On the computer where I am seeing
this, I am also running Kontact and its various components.
Sometime later, I'll try closing Kontact, restarting konqueror and then opening
Kontact. I'm guessing that I'm going to see a problem with Kontact if I do
that.
