I used to have this problem too with a Rage 128 RF AGP. Since I have
downloaded and installed the XFree ati drivers for 4.0.3 found on the
LiViD (www.linuxvideo.org) web site. And now, I only had one or two
X crashes for a month. You can find these drivers here:
http://www.linuxvideo.org/gatos/
at the end of this page, you'll find links to binaries for 4.0.2 and 4.0.3.

Oh, wow! An ATI 128 server that works for the ATI 128 card, that's new to me...:-) I'm also running the 4.0.2 but I've been using the SVGA server since the beginning of support for this card in Linux.
