You mentioned that you'd looked at xinetd - if you run the imap daemon via xinetd, you need to look at the xinetd config for imap, and make sure it has 'flags' set to IPV6. Found the problem, I was trying to set imap flags, not imaps. With flags = IPv6 in the imaps section, I can now access the imap server with both IPv4 & IPv6.
