No, I've never had that happen to me - not on XP, Windows Server (2000-2008) or Windows 7 (never used Vista and avoid 8 like the plague).
You've never gone into XP's safe mode device manager and found multiples of several identical hardware devices, or gone into "Network Connections" and seen "Local Area Connection 2" in a machine that never had more than one ethernet device? If not, count yourself lucky. A BIOS upgrade or reset has certainly done it plenty of times here. So will moving a sole HD from one system to its identical twin, triplet or quadruplet.
