/opt/fax/Mike has attributes of 777 as does the intended fax file /opt/fax/Mike/fax.

Did you restart the hylafax daemon? I do not believe the hylafax daemon is setup to start automatically.
Daemon is running (levels 3 and 5) but not modem action.
jhylafax is still not doing anything, susefax is not working, and kdeprintfax gives the same error as reported above.
Do you have Hylafax's settings in /etc/inittab correct and running?
