Your reply got me thinking, I do a lot of right clicking (filing messages for future reference) and agree it is a real pain.
I don't know of a way to automatically save an attachment, but a button can be added to the toolbar that (I haven't tried it) should allow easier saving of attachments.
Check the Kmail->Settings->Configure Toolbars menu and you will see a "Save Attachments" button is available.
