You will also be replacing the modules that would be needed by that kernel image, so renaming just the vmlinuz file wouldn't accomplish much.
IMO modules from kernel 2.4.19 will not be replaced by 2.4.20. Renaming image and related files in /boot is always a good idea before updating.
