I turned "Smooth fonts" off to improve contrast and readability, but your problems with it prompted me to check underscore and after that how it works in different color combinations that are offered in KDE console (konsole).
I have underscore at size 10 with Monospace, DejaVu Sans Mono [unknown] (which seems to be the same as Monospace) and Droid Sans Mono [unknown], so real culprit could be your desktop settings for smoothing (hinting and subpixel rendering). My hinting is on "slight", subpixel rendering is at RGB vertical.
Smoothing helps a lot with dark characters on a light background. Although, every vertical line is a bit thicker, general appearance is much more improved then without it.
I use "White on Black", white text on a black background, and Droid Sans Mono, and it took some time to try every command in the Profile setup window to find out that otherwise useful "Smooth fonts" is a culprit for hardly readable text.
It seems that automatic change, like when you hover over colors, works with enable - disable smoothing only after using "Apply" button, but I did not analyze to every detail when it works and when not.
Of course other people can come to different conclusions, depending on eyesight and how is smoothing set up on a desktop level.
I found the culprit. I had DejaVu fonts set in Font settings. Changing them to Droid Sans fixed the problem. I am using Droid Sans Mono 10pt. with "Smooth fonts" in konsole. -- Cheers!
