For KDE4, you need to have "kactivities" installed. In the past this was not required, only recommended, so you might not have it under certain circumstances. For Plasma5 you'd need "kactivities5", but this should be installed. You mentioned earlier you don't have an "Create an Activity" entry in the desktop toolbox (top-left). That's correct, but you should have an entry "Activities" that should open the activity switcher where you can create new activities. Does it work with a new user account? Does it work when disabling compositing? If the activity switcher doesn't show correctly it might be a graphics driver problem.
