I don't have the exact same problem, but sometimes plasmashell hangs after a resume, or provides thousands of lines of output with "nouveau:" and some hex number, and some graphics glitches (missing digits in panel clock, ...). Sometimes all 3 symptoms occur, sometimes only one of them
After killing plasmashell and restarting it, everything is fine.

uhm, maybe, but without the crash of the system. And - as Ingmar wrote - it can be solved by killing and restarting plasmashell or by entering initlevel 3 and then going back to initlevel 5. Both alternatives render hibernate/resume useless, though 8-<

Found out that renaming ".config/plasmashellrc" so a new one was created has fixed the problem or at least it seems to have done. Yep, I did the same now. Yesterday I recovered my older openSuSE 13.2 installation, patched the system with all the updated packages and installed the newest kernel from the kernel repo. And say what: No problems at all. Hibernate/Resume works as expected, no distorted screen when running the aforementioned graphical programs. So I'll stay with this system for the moment, too. Generally this is a pity, because kde5 was way faster and I liked the breeze optic.

I had the rendering backend set to OpenGL 3.1 for a while and also had issue but not in your magnitude. Going back to OpenGL 2.0 actually solved most of the Problems
I tried both 3.1 and 2.0 and there were only marginal differences - both gave distorted output in different screen areas. Sometimes only notifications became unreadable with funny coloured stripes, sometimes the whole screen became alarm red or dark black; I even got the complete screen upside down 8-<

What rendering settings did you had choosen in the compositor?
Systemsettings --> Display and Monitor --> Compositor
I had the rendering backend set to OpenGL 3.1 for a while and also had issue but not in your magnitude. Going back to OpenGL 2.0 actually solved most of the Problems
I tried both 3.1 and 2.0 and there were only marginal differences - both gave distorted output in different screen areas. Sometimes only notifications became unreadable with funny coloured stripes, sometimes the whole screen became alarm red or dark black; I even got the complete screen upside down 8-<
This looks much like the bug I reported here: http://bugzilla.opensuse.org/show_bug.cgi?id=939436 Haven't had the whole screen upside down, just the side-mounted task-bar. What puzzles me is how things steadily deteriorated with existing users but a brand new user has been immune - so far.
Besides the hibernation/resume situation for me things get worse the more graphics operations take place, i.e. the more programs I ran which use the graphics adapter a lot.

I've had an unusable Plasma5 system for the past month through graphics breaking up and system freezing. Found out that renaming ".config/plasmashellrc" so a new one was created has fixed the problem or at least it seems to have done. It hints at a system change being incompatible with old configuration files; something that I'm well used to with KDE.
I had similar problems on one of 3 boxes. What I was seeing was full screen "Y" as background, imagine a single qwebyast icon as a dark grey wallpaper and mouseover on anything appearing as little filled squares rather than text. System freezes also did occur. From a reboot the desktop was sane until I clicked on any icon in Folder View which also did the same to Folder View. Removing .config/plasma* did not fix it. As this seems to be a kernel regression, the best, but not easiest, way to find the problem is to use git to clone the kernel source from git://git.kernel.org/pub/scm/linux/kernel/git/torvalds/linux.git and bisect between bad and good versions. The bisection will be easier as you only need to look at changes in nouveau by starting with 'git bisect start drivers/gpu/drm/nouveau/'. If your skills are not up to building and installing your own kernels, then you should report this at http://bugzilla.kernel.org and report it as a regression. The problem started with Tumbleweed snapshot 20150705. In my openSuSE 13.2 system with the latest kernel from the kernel repo I have this problem in very rare cases (some games) only - but this is kde4, while Tumbleweed is kde5. With 13.2 none of the regular system components is causing any journalctl messages like the ones mentioned above and I have no crashes at all. And it seems, that not the kernel or its drivers is the main reason for the problem, but plasmashell and kwin_x11 (or perhaps in a wierd combination with the nouveau driver).
If your skills are not up to building and installing your own kernels, then you should report this at http://bugzilla.kernel.org and report it as a regression.
There already is a bug report, which exactly describes my problem: https://bugzilla.redhat.com/show_bug.cgi?id=1238988 but also no answer.
