Yes, this sounds like worth a bug report. Try calling mkinitrd alone to see if the problem is isolated and reproducible.
mkinitrd generated new /boot/grub/menu.lst but did not include the new
kernel. Basically it was a copy of the previous.

Yes, and also attach perl-BL-standalone.log (or similar log report, don't know exact name). Also please attach detail information about your partition setup, because Warning show that pbl cannot find on disc sdo partition with number 8088 which is really strange.
