I assumed that James' server is not on-line permanently; fetchmail is just used to let DEMON know that it is on-line now. You can use ETRN directly or via a suitable script; I suggest fetchmail as the quickest way to automate it, not the best.
You don't have to be on-line permanently. Demon kindly do the necessary scripting for you to detect whether you're on-line or not. If you are, then they try and push your mail on to you via SMTP. Hence, it's not necessary to use fetchmail at all if you've got a suitably configured MTA, SMTP daemon and tcpwrappers/inetd.conf. This is the way I usually pick up my mail which means that no script is necessary for picking up mail, although for sending mail I have to use a script. I like to pick up my mail this way as it avoids having to configure fetchmail which I personally find a bit of a nightmare aswell as being unnecessary - I never did figure out how to stop it mangling headers.
