It wasn't really an issue with Access Manager or the wikis. However, since this issue kept Access Manager from communicating with one of its own components, I could see some potential for it to have unexpected behavior for already authenticated users. I can successfully review pages in both Chrome and Firefox from two different machines. I was logged in OBS for some time; mostly being idle. Went to wiki and it required from me to log in again. Bugzilla required only to click on "Login" link and I was logged in, which is new to me. Depending on what exactly you were doing, you could hit the timeout for a specific site, but you only need to hit a protected page to be logged back in. Another possible reason is that we just had a whole bunch of changes rolled last week that required a series of new configurations on Access Manager. If you were on the site during one of the configuration changes, it's possible that you could see something like this. I tried the review functionality many times in different ways (also review directly after a login) with different operating systems and different Firefox versions (see my comment#11 and the various other comments) that I cannot imagine the issue depends on the client.
From my non-expert point of view the root cause is something on (or in between) the various servers which are involved to provide the whole openSUSE Wiki functionality.

Most likely not. Although, there is a small difference between your wiki account and mine. I added you to administrator group to test will that make any difference. Just edit some page and try to approve changes. If it fails, then clear browser cache and try again. Today morning I changed this article again (edit and save worked and works all the time for me) but today morning review does not work for me (the usual "Action failed") - all within less than 30 minutes after my login today. I think the issue has nothing to do with the Wiki's permission settings for "jsmeix" or anything else in the Wiki. I guess if I had insufficient permissions in the Wiki, the Wiki would not let me edit or review the article or show me at least a human-readable error message. Furthermore if I had insufficient permissions inside the Wiki, it should fail in any case and not mostly fail but sometimes work (I assume the settings inside the Wiki do not change randomly).
From my non-expert point of view the plain "Action failed" without any human-readable error message indicates that something unexpected somehow somewhere lets it fail.

I clear all except cookies and site preferences. Not just working around in the wiki, but the last time that you actually authenticated to the site? The reason I ask is because this is starting to sound like a problem that we had on the forums. The problem we had with forums was that because almost every part of the site was public, and people were not getting their idle timeout reset. As a result, they would lose their forums session after 6 hours, whether they were using the forums or not. The wiki is set up in much the same way, so it is quite possible that is what people are seeing there as well. There was a pretty simple solution that we used in forums that should work just as well for the wikis. Again and again: With probability 1 this has nothing to do with the client. Look at your servers. What do the log files on your servers tell? Any ideas how to capture what went wrong and why it works after reload. Background: I opened wiki recent changes page from bookmarks. There were some changes to approve and I logged in, went to the first change, review it and pressed Accept, which did not work. Therefore I agree with Johannes that this is nothing to do with login session timeouts. After a shift-reload I was able to accept it. This should be easy to pin down - just look for the "Action failed" string in the source code, and cross-check it the log files between 9:50am and 10am (GMT) today. You will be able to see my IP first failing to accept the revision for Portal:OpenStack/Topics, and then a few minutes later succeeding. The Stage database was recently updated with the Production Content so you should see any posts/revisions that were made as of a few weeks ago.
