MM/NM don't pop up any pin dialog. Also, it's not being detected at boot (bug #449988). If I unplug it and replug it again, I get 3 modems listed in NM. Are you saying your modem has a PIN code but NM never asks for it? That could cause the unknown error (although the modem is directed to give better errors than 'ERROR'). Could you try to send 'AT+CPIN="9999"' (replace the PIN) to the card using minicom and then try to activate it again using NetworkManager.
