You expect this as your root device:
/dev/disk/by-id/scsi-SATA_HDS722516VLAT80_VN643ECDEW7U8F-part11
but your block devices show up as hd* not as sd* devices. hd* devices don't
create any scsi-* device links, that's why it fails.
Any reason to use IDE drivers instead of libata drivers? Did you specify that,
or did the system switch over to IDE drivers and you don't know why?
What does:
grep INITRD /etc/sysconfig/kernel
print?
If you build your initramfs, what does:
mkinitrd
print?
