I think the intended behaviour is a bit different to your expectations or
actually was changed with Firefox 4 as is a bit misleading now.
browser.warnOnQuit is true by default but
browser.showQuitWarning is false
What can be configured by the menu is browser.tabs.warnOnClose which is
respected for additional browser windows apparently but I couldn't find it
working when closing the last window aka quitting the browser. I _think_ that
is intended that way.
So yes, the warning is not supposed to show up w/o flipping the
browser.showQuitWarning preference.
.. unless you are in private browsing mode. In that case no restore and no
warning is happening at all.
Also when the last used tabs and windows are chosen in the startup page General
preference tab no warning is ever happening.
"... and tabs are never restored"
That sounds strange. Restore is only happening when you choose it within the
General preference tab. That works here w/o any problems.
There should also be a button to restore previous tabs optionally if your
startup homepage is about:home
