Regarding the mess I agree, shim-install and its behavior is a mess but currently it all works. If this needs to be changed in some way this needs a discussion before. Even if you fix shim-install to not call grub2-install on efi it will still do that for older versions and I doubt there will be anybody touching it there.
If you experienced the problem that grub-install mistakenly install grub to master boot record in the process of running shim-install, I believe it was a result of target not explicitly specified in grub-install, most likely in conjunction with KIWI image was built on a non efi system.
