Now, I don't know what this is what changes your mode.
It seems to me https://bugs.freedesktop.org/show_bug.cgi?id=94403#c7 addresses your puzzlement: "A recent change to xrandr was not to do a forced query before setting the mode."
This is why I tried to get the KDE folks into the boat as well. Could it be that your: [Module-kscreen] autoload=false doesn't work any more? I don't know.
Kscreen doesn't exist in KDE3. Xrandr from /etc/X11/xinit/xinitrc.d/setup *does* work as expected, as long as the dummy xrandr command preceeds the real one.
