If "Auto Rotate and Center" is switched off, the printout is different if "Portrait" or "Landscape" is selected but the printout is always what the Adobe Reader "Preview" shows - at least for me with a HP LaserJet 1020 (same kind of printer as your HP LaserJet 1018). In Evince with "Auto Rotate and Center" switched on it is similar behaviour. Perhaps there is a bug in Okular? Perhaps there is only a missing feature "Auto Rotate and Center" in Okular? Perhaps your particular PDF in attachment #423966 is somehow not correct which somehow confuses Okular? At least I am confused because your PDF looks like "landscape" but Adobe Reader and Evince seem to "think" it is "portrait" (or is "portrait" perhaps only a hardcoded default which doesn't matter because they also use "Auto Rotate and Center" by default)? But I am not at all a PDF expert to decide this. FYI: Regarding large grey pixels in case of direct PDF printing. I can also reproduce it with my HP LaserJet 1020 when I use the "HPIJS" driver and your PDF. When I use the "HPCUPS" driver, it looks better but the I get the whole first page printed with black background.
