First, I switched from Mutt to Mozilla, but I can still rember my old configurations.
I can't help you with imap accounts, since I have only pop ones.
Consider using fetchmail to retrieve emails from your isp and procmail to setup message filters.
Take a look at the examples enclosed in the man pages of both applications, I've found them very clear...
Mutt configuration is very flexible and powerfull... my personal suggestion is to read the man page... if you want I can send you privately my old mutt's configuration file (comments are in italian, sorry :-)

3) I can see that Mutt can use other mboxes as well. What is recomended for migrating from KMail to Mutt? should I just change the default box and use KMail's? What have others done?

Mutt and Kmail use standard mbox format mailboxes, so you can painlessly use your existing mailboxes with Mutt.
Maybe flags Mutt uses for messages status (read, new, replied...) could be out of sync.
After a successful migration, you can safely delete all the indexes Kmail uses for each mailbox (.<mailbox name> files in you mail dir).
They're hidden files.
