You setup autossh which forwards remote port to local port (e.g. 3389). Then you simply connect to remote port on remote system using remote desktop or whatever.
I made enough progress to start a blog post:

I got it working if I manually type in the ssh line in "cmd".
I ran out of time to troubleshoot the final stage of getting it setup as a Win 7 service.
If anyone has time to figure that, it would be great. I may not have time for a couple days.

I had to make some tweaks, but now have it working and it has survived a reboot of the target Windows 7 PC. No idea of long term reliability yet.
