First off, check whether this volume reset happens in the system level or in PA
level. For example, go to run level 3 without X and PA. Plug in your USB
audio device, and check the volume via alsamixer (with -c option to skip PA
invocation). If the volume reset happens at this stage, try to remove
/etc/asound.state and re-plug the device. If the volume reset happens even
after the removal of /etc/asound.state, it's the device itself who resets the
volume.
If the volume reset doesn't happen in the above test, it's PA who resets the
volume. In that case, removing ~/.pulse might help.
