That creates the file "altmbr.4" which is based on the syslinux "altmbr.bin" generic boot code. It will always boot partition 4, regardless of the active flag.
But that's basically the same as installing bootloader (e.g. GRUB) in MBR, in which case you can also set active flag to any partition. Still there were reports that in this case Windows will refuse updates sometimes.
