OK, I've tried uninstalling pulseaudio, but it's made no difference. In the Yast soundcard setup module, I can play a test sound, but in KDE 4.3 System Settings > Sound and Video Configuration, the test button reports that "SB Live 5.1 [SB0220] (ADC Capture/Standard PCM Playback) does not work" so it definitely seems to be a KDE 4.3 problem.
Do you have the phonon-backend-xine package installed? If using xine moving the ~/.xine folder helps if the cache is messed-up. Also trying with a new user might reveal whether the issue depends on personal settings or not.

Yes, I'm using phonon-xine. I renamed ~/.xine and re-booted the machine (now, where did I learn that trick?) and all is well :) Many thanks to Sven, Scott and John.
