IIRC, when the IBM PC was first released, only the monochrome card was available. The CGA card came out very shortly after.
I don't know when they were comercialized, but I just dug out the "Technical Reference" book of the IBM PC, first edition (Revised January 1983) and it documents both interfaces: IBM Monochrome Display and Parallel Printer Adapter 2-47 ...
