Where is this file? I can't find it on either of my systems at all.

It was in both of the system plus the nic card. Did that it out. But i can't find "mkinitrd"
modules need to be loaded from the initial ramdisk and will delete the initrd. Finish this off with calling lilo and your kernel with builtin SCSI support should boot without problems.
Please give me feedback as it seems that maybe an SDB article is needed for such problems.
