Patches should be extensively commented in the code that constitutes the patch. It is better for the maintainer than a patch tag or a changes entry. The only advantage of using the patch tag is that it can be processed by tools without knowledge discovery in running text. Of course, this also implies that the code itself should be explained in comments, which is not the case more often than not.
