If you could attach /var/log/Xorg.0.log retrieved immediately following discovery that a screen has not recovered after locking, it might provide a clue why. ~/.xsession-errors might as well. Inxi is suggesting you are using multiple DDX drivers (modesetting and radeon). I think it may be mistaken. A look at /var/log/Xorg.0.log would confirm.
