Marcus, thank you for the suggestion -- I'd forgotten about gphoto2.
Unfortunately, gphoto2 (on opensuse 42.1) seems to produce the same
results as dolphin.

I noticed the "--shell" option to gphoto2, and that shows me an
identical directory structure to "-l".
I also noticed the --debug and --debug-logfile options. I'll attach
the output of
LANG=C gphoto2 --debug --debug-logfile=gphoto-debug.txt -l
I'm not familiar with gphoto2 internals, but one thing I did notice
were the debug lines
0.089087 folder_list_func [library.c:5318](0): Duplicate foldername 'DCIM' in
folder '/store_00010001'. Ignoring nth entry.
0.089110 folder_list_func [library.c:5318](0): Duplicate foldername
'DCIM_00000001' in folder '/store_00010001'. Ignoring nth entry.
The line at 0.089087 caught my attention because gphoto sees a "DCIM"
folder, believes there are duplicate instances of the name, but never
lists "DCIM" in the output of -l. (-l lists DCIM_0000001 and
DCIM_00000001_00000001).
