Thanks for the thorough report.
Interesting observations are:
1/ /etc/fstab contains lines like
/dev/md/data /mnt/data ext4 acl,user_xattr 1 2
That seems new. We used to do
/dev/disk/by-id/md-uuid-e3808be8:c94dff04:46f69c0a:25b32886
2/ The names stored in the md metadata is e.g. "linux:data".
This means the array called "data" on the host called "linux".
I suspect your host isn't called "linux" any more (it was during install)
so mdadm won't want to assemble the array as "/dev/md/data" ... unless
it is listed in /etc/mdadm.conf.
Unfortunately you didn't include /etc/mdadm.conf from the root filesystem
in the collected data ... but I guess you cannot think of everything.
Arvin: what do you know about the use of "/dev/md/data" in /etc/fstab?
Would it be possible to add "--homehost=any" to the "mdadm" command when
creating arrays?
It would be best to use the final host name there (rather than 'any'), but
'any' is better than "linux".
