The state of the command is D and top then shows the target lv to be stck with [flush-253:3] (example). Sometimes, this means that rebooting hangs. Sometimes it can reboot. The last time I moved a 141MB file from /home/roeland to /srv/www/htdocs and the system was stuck with flush-253:3. Obviously this didn't work out. After restarting the target file was empty. The bad part here is that it's difficult to replicate th eproblem. The server itself is a debian based host running kvm; /dev/vda is the disk image it runs on. Other copy commands to different lv's don't seem to be affected. E.g. only (in this case) /srv was stuck.
