yes

2) What KDE version was on the 11.4 install?

4.6.0

3) How do you start it from the kicker menu?

by searching for 'networkmanager' in the start menu

4) Can you add it to the system tray by clicking the expand triangle, System Tray settings, Extra Items, Network management?

Indeed, that works.
