I can confirm related issue with nouveau on Geforce6600 (without xorg.conf) and
intel on i915 (dualheaded with basic xorg.conf). Looks like similar with

Opensuse 11.1 works OK, but 11.3 not honor any xorg configuration and always
set 96dpi.
with strings in log:
(**) NOUVEAU(0): Display dimensions: (377, 301)mm
(**) NOUVEAU(0): DPI set to (86, 86)

(II) NOUVEAU(0): Setting screen physical size to 338 x 270
To reproduce, try set "DisplaySize 377 301" option in
/etc/X11/xorg.conf.d/50-monitor.conf and start xinit without kdm.
If I change DPI with `xrandr --fbmm 377x301` in running session, all the rest
applications looks good. So Ill disagree with beginning sentence "Passing
neither --fbmm <xvalue>x<yvalue> nor -dpi 96 has an effect."
However I need to use workaround in Xsetup again for anything different from
KDE/Gnome. Or set DISPLAYMANAGER_KDM_LOCALARGS since krandrtray have no ability
to change DPI and save settings.
