I cannot see the problem in the logs: Two RAIDs and one VG are discovered.

Then a proposal is created which includes partitions on sdd and the
LV with swap. But e.g. sda3 is not present in the logs.

Maybe you originally selected to create a proposal on sda and that created
sda2 as extended partition and several logical partitions. That is the
proposal I get on my test machine (which only has two disks and thus cannot
use sdd but must delete on of the RAIDs).

The proposal does not reuse existing MD RAIDs.
