lets return to the inital issue.
An strace shows this:

Looks like that mmap causes the Fatal server error
If legacy_mem access is really the error, then the code is ia64 specific
because only ia64 can have such a file in sysfs. Look for the HAVE_PCI_LEGACY
define in kernel-source, ./arch/ia64/include/asm/pci.h and
/drivers/pci/probe.c
Can you check whats up with that mmap() call?
