Hmm, again it seems like you *probably* have a swap entry in the page tables of
a vma that should never contain swap entries (a file vma -- file mappings don't
get swapped out as such, they just get written back to the file and reclaimed).
This suggests some corruption has happened somewhere, but it is really hard to
tell whether it is due to bad hardware, or some unrelated software bug causing
the memory scribble.
Unless the bug is readily reproducible, it is really hard to ever work it out.
We could detect the corruption earlier, or even perhaps correct the corruption,
but it would still not tell us what caused the corruption. However I guess
hardware errors are usually single-bit errors, whereas software error would
normally overwrite the whole byte with garbage. Hmm, I'll see how difficult it
will be to do some form of software-ECC on page table entries.
