Two suggestions:
1. Confirm that you've added the correct module. For example, I had to add "php4" to the APACHE_MODULES variable in /etc/sysconfig/apache2, NOT "php".
2. If the above is correct, try adding "info" to the APACHE_MODULES variable in /etc/sysconfig/apache2. Then activate the "/server-info" page and give yourself permissions to access it. Something like:
<IfModule mod_info.c>
