YaST keeps its own idea of the contents of each package repository you configure into it. Since the contents of many of the repositories changes over time, YaST's notion of that content must be updated. "Refreshed" in its terminology. In the Installation Source you can configure each source to be automatically refreshed each time Software Management is launched (*). That's the "Refresh" column. You can also perform one-time source refresh operations there (select a source, click on the Edit button and select "Refresh"). (*) This is new in 10.0. It's easy to forget to refresh and it often lead to the frustrating experience of looking for a newly announced package or packages and seeing nothing.
