On a new installation of 11.4 64 bit KDE desktopn with an M Audio 2496 soundcard I am not getting any sound output from any apps. neither do system sounds work. I have checked the mixer settings which are ok. and I can play a volume test from Yast and sound examples from the multimedia section of KDE system settings. The same box also has 11.3 installed and the sound is OK there. Whether Pulse Audio is enabled has no effect neither does running as root or another user.
