Sorry, no. Just another data point. I'm running lvm on Leap 15.0 and have the latest versions of lvm and libc too. The only segfaults on my system are firefox! :( But I don't install new kernel modules very often at all. One other likely difference is that my kernel is not completely up-to-date. Since the error is unmapped memory, maybe there's been some change in that area of the kernel? Can you run lvm under gdb to get more information?

openSUSE Leap 15.0, all updates applied, system is using LVM on four harddisks...
