The "File Associations" does have Okular at the top and Okular is installed as is GIMP. I would have thought this would start Okular but it starts GIMP. To change the behavior I had to do it in Thunderbird itself by expanding the attachment (>) as I mentioned before and picking "Other" and then navigating to /usr/bin/okular and selecting "Do this automatically..."

That did the trick! Funny thing is, I had tried that before, but the "Other" option choice on opening the attachment never showed up before. For some reason it was always just "Gimp", and there was no "other" option. When I did it this time, the "other" option showed up. I might also mention that in previous versions troubleshooting this problem included deleting the "mimeTypes.rdf" file under the folder ~/.thunderbird/<profilename>.default/. I guess in thunderbird 9 they are not longer using the "mimeTypes.rdf" file, because it doesn't seem to exist on my system.
