From the kmix handbook. You need to click on "Restore Volume on Login".
Restore volumes on login Let KDE restore the volumes when you Login: This restores your personal volume levels, stored when you last logged out. If your Operating System saves the volume levels, you might not need this option (but on a computer with multiple users it is still needed). A new install of 10.1 wants to reset the sound volume to the midrange (which I cannot hear). I reset it to maximum in Yast so that I can hear it but when I boot, the volume is at or near the midrange. On sound the only thing that is constant is the I cannot hear it. v9.2 did not do this.
